“I just wanted to share some of my favorite patient success stories we’ve had so far with our CCM program. My personal favorite is a 55 year old male who started with us around 6/2021. We’ve lowered his blood pressure from 200/100+ to 125/70 mmHg, increased his renal function, and ultimately lowered his 10-year-ASCVD-risk from ~40% to ~5%. I’ve also worked closely with a patient with dementia and their family to navigate the best medications to improve their quality of life, such as deciding between scheduled vs. prn quetiapine or hydroxyzine.
These are examples of some of our biggest wins. I believe we’ve had many preventative wins as well. I’ve been able to serve as a resource to help patients self-monitor more by ensuring our patients have blood glucose strips, lancets, machines, and know how to log their blood pressure/blood glucose. I’ve also been able to help one of our patients navigate a recalled CPAP machine to prevent further complications in the future. I’ve discussed/administered any needed vaccines (COVID, flu, shingles, pneumonia) to all of our patients, and I screen all of my diabetic patients for foot and eye exams, as well as statin use, hopefully preventing future unnecessary events.”
