If you are interested in providing the vaccine for your patients or staff, the first step is to complete the COVID-19 Vaccine Provider Intent Form.

In addition to the VDH Vaccine Provider Intent form, the CDC Vaccination Program Provider form must be completed as well. That link can be found at:

The Provider Intent Form cannot be saved and must be completed all at once. Consult the Instruction Guide before you begin to preview the required information.

  • The Pfizer vaccine received temporary approval for emergency use in the UK on December 2, 2020. Emergency use authorization in the United States could occur as soon as December 10, 2020.
  • The Pfizer vaccine will have rapid production scaling, which could potentially produce tens of millions of doses in 2020 and 1.3 billion doses in 2021 (globally).
  • Pfizer has a supply agreement with the U.S. government for 100 million doses, with the potential to increase to 500 million doses pending the vaccine receives full approval. The U.S government will provide this vaccine for free.
  • Pfizer is utilizing thermal shippers with GPS and temperature monitoring to ensure shipment directly from manufacturing site to vaccination site maintains necessary temperature to ensure safety.
  • Undiluted vials can be stored in refrigerators at 2-8 degrees Celsius (35-46 degrees Fahrenheit) for 5 days. Diluted vials should be used within 6 hours of dilution.
